You will receive an email registration form to fill out prior to your visit. Please fill it out as completely as possible, including the baby’s Dr information. The consent and form information MUST be completed 25 hrs prior to your visit, unless we are working you into a last minute visit. Make sure you click to accept the consents. You can add your insurance info on this form. Please mark if it’s self pay or insurance. You can use a mobile phone but many report it easier on a laptop or computer. This helps give you personalized care specific to your unique needs. Please make sure you put the reason for your consult under “parent concerns.”
Please write down any questions you want to have answered. Complete the pediatrician information as well.
Try to feed your baby 1.5 – 2 hours before the scheduled consult. I will bring a lactation scale to measure your baby’s intake at the breast.
Have your baby’s weight history available.
Have your breast pump assembled and set up if you have questions about your pump or flange fit.
Please put any pets away in another room while I am there to prevent any issues if they are going to be a distraction.
At Tennessee Lactation Care, our goal is to give you and your baby the best care possible. To help us do this, we use Artificial Intelligence (AI) technology to take notes during your visits. This helps us focus more on you and less on paperwork, making sure your care is complete and accurate.
We do have a 24 hr cancellation policy. If you are not available at the time of your scheduled visit and do not cancel 24 hrs prior, there is a $25 rebooking fee.
Aetna/BCBS/UHC
I will bill your insurance for you, but you will be responsible for anything your insurance doesn’t cover. Please make sure you’ve verified lactation benefits with your specific insurance plan so you’re aware. Once my biller submits the visit claim we’ll know if there’s a balance. Please sign the insurance billing agreement sent to you – also on our website.

AFTER VISIT:
If you have any questions after the visit, you may email, text or use secure messaging within your Parent Portal to communicate after our visit. I will get back to you as soon as possible. I will do my best to get back with you within 12 hrs or sooner.
If you have been approved for visits through The Lactation Network, they will handle all insurance claims. You will NOT receive a bill, so disregard any EOB statements that say you may owe a portion. The Lactation Network will not bill you. You are 100% covered for 6 or more sessions.
